REAPPLICATION INSTRUCTIONS FOR

EXAMINATION

It is the applicant’s responsibility to compile and submit all documentation required.  Failure to properly fill out and complete the application forms may result in determining ineligibility for an examination.  

WHO SHOULD USE THIS APPLICATION 

Individuals reapplying for an examination 

Note: Please complete an initial application if one of the following applies to you: application was in incomplete or denied status; or did not submit a reapplication form within the last five years.

 HOW TO REACH THE BOARD  

APPLICATION DEADLINE: Applications must be postmarked by the deadline.  If applying for the Spring examination, the postmark deadline is December 1st.  If applying for the Fall examination, the postmark deadline is June 1st.  Please refer to Oregon Administrative Rules (OAR) 820-010-0215, and (OAR) 820-010-0440 for further details.
